Weak nystagmus in the dark persists for months after acute unilateral vestibular loss.

Recent research by Chen et al (Front. Neurol., 15 December 2023) found that 96.2% of cases exhibited paralytic nystagmus toward the contralesional side, indicating its clinical significance as an indicator of the lesion side.   


For patients presenting with a history of dizziness weeks to months prior, careful examination for weak nystagmus, especially with fixation removal, is crucial. Subtle nystagmus in the dark can persist for months and according to this study, should not be overlooked in patients with chronic dizziness.
